Quiz With Images as Radio / Next Page In Gravity Forms WordPress

I am trying to think of a way to accomplish this in Gravity Forms (or another form plugin).

- There is one question per page. There are multiple images that represent the answer to this question.

- When the image button is clicked it records the answer selected, and goes to the next page (question).
