Plugin for survey that shows graph as a result WordPress

I would like to make a survey with 50 questions divided in 5 categories. The questions have to be answered by scoring on a scale from 1 till 10. On the result page the sum per category needs to be shown, preferably in some sort of graph. I would also like to get a report with the answers to all the 50 questions. Does anyone know of a Wordpress tool that supports these kind of features?
