Help getting a plugin to activate and work WordPress

I'm trying to get the WP Server Status plugin working to show our employees which servers are up or down. However the plugin does not seem to activate. Any assistance would be great.

That's the problem it doesn't activate.

It is activated and widget is available and also setting is also available under setting menu , please check
