Custom Search for Wordpress theme WordPress

This is my first post here so bare with me if its not asked in the most appropriate way..

I am working on a Custom Wordpress theme and need help with creating the search functions based on custom fields. The user can choose to search via 4 options in a select/drop down box (name, age, height or eyes) based on the selection, a second select box or input field will them appear beside the original select box. For instance, if the person search by age, the second select box will populate with 12-17, 18-35, 36-40.. and so on. But if people search by name , an input box will need to appear next to the select box instead.

I need help with created and adding this to my template, and for the search to function correctly from the custom fields ( the name option is the post title)

I have a online development area where I can allow someone to login and create the functionality online. I would also be available on skype or email to discuss this functionality also..
