Can you add this fields and feature to the form you made for me?
A madatory checkbox at the end
recaptcha, or captcha or any captcha system
register user to a new group (partners) so that after he can modify is form
keep form fields informations after susbmission failed.
What I have done alone since last time:
styled the form
add news fields
add nonce to the form
